FiveStar collects these details to assess your application, prevent fraud and, if approved, administer agreements, tax records and payouts. Required fields are needed to assess and operate your FiveStar Affiliate account. We use contracted hosting, email, security and payment providers, including overseas providers described in our Privacy Policy, which also explains retention and your access and correction rights.

Submitting securely stages these details for review; it does not create or activate a FiveStar Affiliate account and does not record agreement acceptance. If approved, FiveStar will set the exact commission terms and email the complete agreement for you to review and accept.
